Can a donut mastopexy correct tubular, tuberous or constricted breasts?

Jun 7, 2018 | Breast

Constricted or tubular breasts tend to be too narrow with a breast crease that is too high. Tight ring-like bands of tissue around the areolae and nipples may create the appearance of protruding or puffy areolae. A higher than normal breast crease may make your nipples appear to be too low or down pointing. Constricted breast features can be very mild or severe. In fact, mild variations are relatively common. To summarize, the features can include:

  • Inframammary fold (breast crease) too high
  • Inframammary fold misshapen
  • Breasts too narrow (disproportionately narrow base width)
  • Insufficient breast skin
  • Constricted or tubular shape, resulting from tight bands of tissue within breasts
  • Protruding or bulging appearance of areolae (pigmented area around nipples)

Improvement is possible and generally includes a breast augmentation, sometimes combined with a lift (mastopexy). The lift is sometimes limited to an incision around the areola. During the procedure, Dr. Jaime Schwartz MD FACS releases the tight, constricting bands from the inside, allowing the skin to expand properly.

This can sometimes include lowering the inframammary fold to a more ideal position and improving its shape. Breast implants help create the right amount of volume and improve breast contours, resulting in a more rounded or teardrop shape. This can be possible to resolve with fat grafting in milder cases.

WHAT TO KNOW ABOUT THE DONUT MASTOPEXY TECHNIQUE

A donut mastopexy is a lift that uses a circular incision around the areola to tighten breast skin, center the nipple, and improve breast shape with minimal external scar. The surgeon removes a ring of surrounding skin, releases tight breast tissue bands, and elevates the areola to a more natural position. This technique preserves sensation and blood supply to the nipple in most patients and can be paired with augmentation for additional volume.

A standard breast lift often includes vertical or anchor incisions for larger lifts. A donut lift limits scarring to the areola, which suits mild to moderate sagging. It is not designed to correct severe sagging breasts or require drastic skin removal. When mild constriction or areolar stretching is present, a mini breast lift with a donut lift pattern can provide a firmer look without drastic changes.

The donut lift procedure allows the surgeon to score and release constricting tissue, lower a high inframammary fold when needed, and reshape the base width. Some patients add breast implants or fat grafting to expand the lower pole and improve symmetry.

WHO ARE GOOD CANDIDATES FOR DONUT MASTOPEXY?

  • Women in good health without active diseases.
  • Stable weight and realistic desire for a natural lift.
  • Mild to moderate sagging or shape irregularity, with nipples near the breast crease.
  • Completed pregnancy and breastfeeding, or understand future pregnancy may change results.
  • Do not plan more children in the near term.

A consultation with experienced plastic surgeons determines whether breast lift procedures like a donut mastopexy alone, or combined with breast augmentation, best meet goals.

WILL THERE BE SCARRING?

Because the incision sits at the areola border, most patients notice the scar blends with the natural change in skin color. The areola can be reduced in diameter and secured to resist areolar stretching. Scar care and sun protection help maturation.

HOW LONG DOES A DONUT MASTOPEXY LAST?

Results typically last many years. Skin quality, weight stability, pregnancy, and gravity all affect longevity. Patients who maintain stable weight and good skin care often enjoy a long-lasting lift. Significant weight changes or pregnancy can reduce support and create new sagging.

HOW MUCH DOES IT COST?

Pricing varies by region and complexity, whether implants or fat grafting are added, and facility fees. A standalone donut lift is generally less than a full anchor lift. Adding breast implants or combined breast procedures increases cost. Personalized quotes follow examination and surgical planning.

SAMPLE RECOVERY TIMELINE

Recovery specifics can vary depending on the nature of the procedure and individual healing factors. The following timeline provides a general overview of what to expect after a donut mastopexy, but your surgeon will provide personalized guidance tailored to your unique situation.

  • First week: Swelling and bruising typically peak; patients should focus on light walking and avoid strenuous activity to support healing.
  • Weeks 2 to 3: Sutures begin to dissolve; most patients are able to return to desk work and light daily activities while continuing to avoid heavy lifting.
  • Weeks 4 to 6: Gradual reintroduction of exercise is encouraged, but it is important to avoid any strain on the chest area to protect the surgical site.
  • Month 3 and beyond: Scars gradually flatten and lighten in color; the final breast shape settles as swelling fully subsides.

Following all post-operative plastic surgery instructions carefully is essential to protect the lift and achieve the best possible results.

LIMITATIONS

A donut lift will not correct severe sagging breasts or very low nipple positions as effectively as extended patterns. Patients who require drastic elevation may be better served with a standard breast lift. Your surgeon will recommend the technique that meets your goals with the least scar.
